Every time entry can carry a note describing what the time was spent on. Notes appear on the timesheet, in detailed reports, on client reports, and on an invoice when you bill tracked time — so they are usually the only record of what was done, as opposed to how long it took.
Owners and admins can require them: Settings → Require notes on time entries.
With it on, an entry cannot be saved without one. That holds everywhere time is recorded — typing an entry in the timesheet, stopping a timer, asking the assistant to log time, and integrations posting over the API — so there is no path that quietly skips the rule.
Two things it deliberately does not do:
There is no per-project or per-task version of this setting today — it is on or off for the whole workspace. Where only some work needs describing, the usual approach is to leave the setting off and use the approval step: a manager returning a week is the point at which a missing note gets asked for.
The Detailed time report lists notes alongside date, person, project and task, and exports them to CSV. On a project, expanding a task shows who logged the time; the hours link through to those entries and their notes.
